Order Process 

Contact the Xbox customer support call centre on Toll free 1 800 555 741 to confirm disc replacement availability, register the replacement order, pay the replacement charge (by credit card only) and obtain a call centre order request number.

You will then receive an Xbox 360 disc replacement order form which must be submitted with your damaged disc and include your order request number.
